My driver history record contains my individual driver history information, but is not a certified copy. This record does NOT include any complete,
personally identifying information, such as photograph, address, social security number, telephone number, or driver’s license number.
I understand that backgroundchecks.com uses an online process that requires me to prove who I am. As a part of that process, backgroundchecks.com records my
internet protocol address for my computer. I understand that backgroundchecks.com will not refund the fee I am paying if the online tool is unable to verify who I am.
I understand that I must instead follow backgroundchecks.com’s instructions on how to prove who I am by providing copies of records. I understand that until I do
prove who I am (either through the electronic tool or through copies of records), backgroundchecks.com will not process my order.
I also understand that, under the Uniform Electronic Transaction Act or the Electronic Signatures in Global and National Commerce Act, I may sign this form electronically.
